Well, since there's about a 95% certainty that Jacoby dug the
locket up, and since any scenario which doesn't involve Jacoby
digging the locket up requires that the Tooth Fairy came down
and waved his magic wand to make things happen, I guess we can
assume that Jacoby somehow crawled out of ICU, scrounged up a
silenced pistol, dragged himself to the Great Northern, and
pumped three shots into Coop for no apparent reason.

There is nothing we've seen in the series to justify either of
these assumptions.
