The way I understood her voice tone, was that it was a combination of
dismay and outrage that Leland had just attacked her, and that he was
so obviously around the bend.  Sarah's been worrying about her husband
for some time.  Crying out his name at that moment is like a culmina-
tion of all that worry--her psychological pain and her physical pain
coming out in the same wrenching "Leland!!"

This is a little off the wall, but even not having read the diary and
thus theoretically not knowing about Laura's horse, I was struck by the
way that Sarah seemed to recognize the horse--she was amazed and even a
bit pleased to see it.  And the facial expression on the horse was kindly,
even affectionate.  (Those who know horses well may differ with this
interpretation, though--but they *do* have definite facial expressions.)
This led me to the conclusion (here I'm stretching...) that the horse
was a horse that Sarah had known quite well--therefore most likely a
horse either of Sarah's or Laura's--who has been dead for some time--
and that what she was seeing was the *ghost* of this horse, who appears
at her moment of extremity (whether on the verge of death or not, she
certainly was _in_extremis_ emotionally) to comfort her, perhaps even
to guide her to the other side.  It could well be that the constellation
of spirit-world events that was allowing "it" to "happen" (as per the
Giant's commentary) again, includes a thinning of the veil between the
flesh and spirit realms, so that benign ghosts, unrelated to Bob or
Mike, might be appearing all over town.  And after all, if the veil was
thin *anywhere*, it was thin in the Palmer house that night!
